I was playing 10.1 for a few months now and I just started using equipment like Behemoth Sledge mainly and I put it on my creature and my creature got destroyed the equipment went to the GY with the creature..
Why does this happen sometimes? It sometimes stays but rarely..
How do we fix this?

Well, the answer hasn't changed from all the other times this question has been asked (which you can see by using the forum search on "equipment" and "graveyard"). It's a bug in Wagic 0.10.1, it will be fixed in the next release. The only fix is to upgrade when the next version of Wagic is released (or to download and compile the source code of the SVN version, which already has this fix).
