Questions about iphone factory unlock

Ive noticed that AT&T has a form on their web site and t-mobile has a customer service line you can call to have your iphone factory unlocked if you meet the requirements. My question is does the carrier charge you to use this method, if so how much? And what is the turn around rate the time I call or fill out the form to the time the request is fulfilled or denied?
